US Vaccination Requirements

Documented written proof of immunization is required in order to fulfill the requirements of the U.S. Immigration and Nationality Act. These are the immunizations required even though they may not correspond to what your doctor or public health department feel are necessary.
The Panel Physician has no authority to recommend an exemption from the required immunizations unless there is proof of a valid medical contraindication signed by your doctor. The Panel Physician will accept a verbal declaration that you had chickenpox. However the Panel Physician CANNOT accept a verbal declaration for the other immunization requirements.

Vaccination Requirements (by age): Please see the drop down menu for required vaccinations according to age.

Please copy and print out and take to your doctor if you are unsure that you meet the requirements to receive any vaccinations prior to having your medical assessment.

COVID-19 vaccination is now a requirement. You must show full vaccination to be eligible to travel to the US.

Effective July 25, 2022, the lower age limit for the COVID-19 vaccine requirement is 6 months of age. Any applicant 6 months through 4 years of age who presents for a medical examination on or after this date will be subject to this requirement.
